Why atopic dermatitis is typer I hypersensitivity rxn and contact dermatitis is type IV. I have hard time differentiating the two of them.
Atopic dermatitits vs contact dermatitis?
atopic dermatitis is type 1 because is IgE mediated. Allergen are processed by antigen presenting cells that interact with CD4 TH2 causing IL to stimulate B cell maduration. Plasma cells switch from IgM to IgE. Now the IgE ab are bound to mast cells, so when the angingen presents again, its going to cause degranulation of mast cells. Contact dermatitis is antibody INDEPENDENT T cell mediated, (type IV). Now, one clue is onset time. In atopic dermatitis occurs in mins, contact dermatitis is a DELAYED reaction hypersensitiviy, so it can occurs in days (the onset of symptoms). (That's one of the clues.)
